Practically there's very little difference: R means no unaccompanied kids, NC-17 means no kids period.
In perception there's a huge negative stigma attached to NC-17 and studios will go to great lengths to avoid it (not out of prudishness, but because NC-17 is box office death).

The ESRB has a similar "mark of death" rating for video games: AO ("Adult Only"). AO games are very rare, not only because no retailer will sell them, but also because if you're making a porn game, you're not going to bother with getting it rated and will just slap it up on the internet. To this day, the only game rated AO to get a worldwide release is Grand Theft Auto: San Andreas, and that's only because the Hot Coffee saga caused the ESRB to re-rate the game to AO post-release, and then revert the rating once Rockstar released a patched version of the game.

I've been watching Hannibal on Netflix and it has so much blood and gore that I thought for sure it was a Netflix original series. Turns out it was actually on NBC.

Well... In Germany we have ads with boobies.
This is an ad for a liquid shower bath soap from the 90s (and the boobies are at the end).
Also cursing is not a huge deal here. There were times when talk shows (the bad sensational ones) (but these shows are essentially dead) during the day were bleeped but nobody gives a damn for a long time. They have lost their edge with repeated use.
Cursing around children is still considered rude here.
However violent films get often censored unless they run very late, after 10pm. I haven't seen TV extensively for a long time. I use my TV for Youtube and Netflix and for tuning in to the news.

Yeah, I'm aware boobs aren't as taboo in most other countries than in the US. It wasn't unusual to see boobs in anime (even those aimed at younger audience) when I was a kid, although I think nowadays even they're cracking down on it unless it's aimed at adults.

Well, TIL there's a name for this phenomenon: Scunthorpe problem.
I had the problem recently with my website. Somebody's comments kept getting flagged for spam, so when I looked into it it's because the filter I was using filtered out email addresses with ".ru", but the guy has a firstname.lastname email address, and the first two letters of his last name is "Ru".
